Eligibility Criteria

You are entitled to a free NHS eye test at home if you are unable to leave home unaccompanied to visit your high street opticians, as well as being able to answer "yes" to any of the following

  • 60 years of age or over?
  • Diagnosed with diabetes or glaucoma?
  • Considered to be at risk of glaucoma (as diagnosed by an ophthalmologist)?
  • 40 years of age or over and the parent, brother, sister, son or daughter of a person diagnosed with glaucoma?
  • Registered blind or partially sighted?
  • Eligible for an NHS Complex Lens Voucher (your optician will advise on your entitlement)?

You are entitled to an NHS optical voucher/ NHS spectacles:

  • If you and/or your partner receives Pension Credit Guarantee
  • If you and/or your partner has a valid HC2 certificate (certificate number and expiry date required)
  • If you and/or your partner has a valid HC3 certificate (certificate number, expiry date and amount that voucher will be reduced by, are all required).
